Standard Application Checklist

Proof of Citizenship
Bring an original or certified copy of your birth certificate, naturalization certificate, or previous passport book.

Proof of Identity
A valid driver's license, government ID, or military ID. Please bring a photocopy of the front and back of this ID.

DS-11 Application Form
Completed but NOT signed. You must sign the form in the presence of an authorized agent at our center.
Note for Minor Applications:
Both parents must be present with the child and have their own valid photo identification. If one parent cannot be present, a notarized Form DS-3053 is required.
